DREST’AGATH
This boss fight has no phases, rather it is a continuous stream of adds spawning to allow you to gain the ability to damage the boss itself. Drest’agath heals 15 million HP every 5 seconds, and only damage taken by players with the Void Infused Ichor debuff cannot be healed by this constant regeneration.
The fight is entirely scripted however. Adds will spawn in roughly the same place each time, making the fight more of a challenge of pre-planning and damage control rather than a gear check.
As such, we can plan ahead for each pack of adds for positioning, and here is a map of movement and add killing priorities courtesy of Rivalry, based off of the Stankie guide below.

Summary
To summarize these pictures, it is important to know that is fight isn’t mechanically challenging in any way, it is an exercize in patience and consistency.
Grabbing Ichors in a set order is an option, but most guilds agree that it adds a needless layer of complexity. It is important you regulate your own Ichor usage around the biggest burst of damage you can do to the boss in a 30 second window. You cannot pick up an Ichor for 1 minute after your previous has expired, so it is a balancing act of finding the best time to grab an ichor and deal as much damage into the boss each minute. Note that ichors do not sit on the floor forever, wasting them is not an option.
Add management is crucial to the fight so we do not get overwhelmed. As you’ll have noticed, each picture specifies killing adds before eventually taking ichors and burning the boss when most of them are dealt with. This will of course depend on if the adds die fast enough, as we do not want ichors to despawn.
It is important to keep the positioning stated in the pictures, and to stack whenever necessary and to avoid the cascade of crap on the floor that is far more frequent than on Heroic. Each add casts their special ability whenever one other add dies, so they are abilities you will definitely see on Mythic, and might’ve witnessed on Heroic.
Tentacle of Drest’agath – Reality Tear: Whenever an add dies or the boss casts Throes of Agony (100 Energy), each Tentacle present will start casting Reality Tears in a line across the entire room. The direction of these Tears is shown by a large arrow pointing out of the tentacle.
Eye of Drest’agath – Errant Blast: Whenever an add dies or the boss casts Throes of Agony, each Eye present will start casting Errant Blast, which causes red swirlies to spawn over the encounter floor, casting down huge clumps of gore onto floor dealing lethal damage to anyone hit.
Maw of Drest’agath – Spine Eruption: Whenever an add dies or the boss casts Throes of Agony, each Maw present will begin channeling Spine Eruption. This causes a large circle of spikes to spawn around each Maw add and shortly after pierce anyone standing ontop of them for lethal damage.
